One of the most common questions we hear during TMS therapy is: How do I know if it’s actually working? The answer isn’t always as obvious as you might expect. TMS treatment for depression usually works gradually. You may not wake up one morning suddenly feeling completely different. Instead, the first signs of improvement can show up in everyday life, sometimes before you even recognize them as progress.

Maybe you’re getting out of bed a little easier. You finally answer a text you’ve been ignoring. You make dinner instead of ordering something because you’re too exhausted. You take the dog for a walk, get through a full workday, or realize you actually enjoyed a conversation with someone. Those small changes matter. We Look at More Than “Do You Feel Better?”

Before beginning Transcranial Magnetic Stimulation (TMS), your care team establishes a baseline of how you’re feeling and functioning. Throughout treatment, we continue checking in and comparing where you are now to where you started.

That can include standardized depression assessments, such as the PHQ-9, along with conversations about your symptoms and everyday life.

We may look at changes in:

  • Mood and overall outlook
  • Energy and motivation
  • Sleep
  • Concentration
  • Interest in activities
  • Social interaction
  • Ability to keep up with work, family and everyday responsibilities

A questionnaire gives us useful information, but it doesn’t tell the whole story. Tracking TMS progress also means paying attention to how life is actually feeling outside of the treatment room.

Sometimes Other People Notice First

This happens more often than you might think. A spouse might say, “You’ve been talking more at dinner.” A friend may notice you’re making plans again. Maybe your kids ask you to do something and, instead of immediately feeling overwhelmed, you actually want to join them. Depression can slowly change the way we live, so improvement can be gradual too. These real-life changes help your TMS care team understand how treatment may be affecting your daily functioning, not just a number on an assessment.

What If You Don’t Notice a Difference Yet?

Don’t judge your entire TMS treatment by one day or even one week. Everyone responds differently. Some people notice changes relatively early, while others don’t experience meaningful improvement until later in their treatment course. We break down the timeline in more detail here:

The important thing is to keep communicating with your care team. Tell us what feels different, what doesn’t and even what you’re unsure about. That information helps us follow your progress throughout treatment.
